Physiotherapeutic procedures in the treatment of postpartum stress incontinence

This bachelor thesis focuses on physiotherapeutic procedures in the treatment of postpartum stress incontinence. It is divided into theoretical and practical parts. One of the aims of my thesis is to describe the therapeutic approaches that can be used to treat postpartum stress incontinence. Then, in the theoretical part, I focus on describing the postural changes caused by pregnancy and the period during the postpartum period. The last aim is to describe the effect of the chosen therapy on the woman's posture and the impact on quality of her life. In the theoretical part, information on the current state of the problem is given, as well as the anatomy and physiology of the organs in the body. The following chapter deals with stress incontinence, specifically its classification, epidemiology, etiology, diagnosis and treatment. The next chapter deals with the changes on a woman's body during pregnancy, and last but not least, there is a chapter discussing pelvic floor problems and the possibilities of physiotherapeutic procedures to affect (not only) the pelvic floor muscles. The practical part describes the methodology of the work, the examination content and the results are noted in the form of case reports. In total, there are case reports of 2 probands, who were at different times after childbirth and suffered from different degrees of stress incontinence after childbirth. For 10 weeks the probands practiced the given exercises, a description of which can be found in one of the chapters. At the same time, their examples can be seen in the photographs in the booklet, which is included in the appendices. The work can be an inspiration for physiotherapists who work with the pelvic floor area or specifically with women after childbirth. It can also be useful to anyone dealing with a problem in the pelvic floor area. The booklet can be the first step in solving it, as it contains educational information as well as basic exercises for the pelvic floor muscles.
The comparison of the effect of methods by Kegel and Mojžíšová in patients with stress incontinence

Urinary incontinence is defined as the involuntary escape of urine that is objectively demonstrable and causes social and hygienic problems. It is a common problem, that is in its results also important medical, social and economical defect. The object of the research of this work was a group of woman patients with the stress form of urinary incontinence, that was instruated by Kegel's or Mojžíšová's methods in home exercises. The goal of this thesis was to influence contractility, power and tense of pelvic floor muscles and to evaluate the effect of the chosen methods. Objectively the results of the work don't demonstrate change of assessed parameters, but it has came to subjective improvement of urinary incontinence's symptoms and differences in kineziological investigation. The Bachelor's thesis deals with a pelvic floor dysfunction with focus on the stress incontinence. It describes the influence of pelvic floor dysfunction on the deep stabilization system of the spine, the urinary tract function, the sexual function and partner and social life of the patients. In the Bachelor's thesis are summarized available information on the pelvic floor dysfunction and the stress incontinence. In the practical part are processed three casuistries of patients with the stress incontinence occurrence after two or more deliveries. The casuistries focus on complex kinesiological examination, including palpation vaginal examination pursuant to the PERFECT scale, diagnosis and course of the therapy and individual approach to the patients. The thesis adverts also to the quality of life change and the fact that the pelvic floor dysfunction can manifest itself at any age.

The effects of physiotherapy for female stress incontinence in working age

The bachelor thesis deals with impacts of physiotherapeutic methods on woman stress incontinence in working age. Stress incontinence is an uncontrollable leakage of urine during the increase of intrapelvic pressure in activities such as sneezing, coughing, laughter, lifting of heavy weights etc. Middle-aged women are mainly affected by this symptom.The first part of the thesis refers to the anatomy and physiology of the urinary tract, pelvic floor and core stabilization system, which the pelvic is part of. Subsequently, the incontinence and its distribution is outlined, focusing on stress incontinence and its Etiology, diagnosis and treatment. Since the stress incontinence has an unpleasant experience for most of the women, it is also mentioned its negative impact on the quality of life. The second part of the thesis is practical, using qualitative research in the form of three casuistries. Patients are three women aged from 42 to 57 years old suffering from stress incontinence. The goal of the thesis was to point out the effect of physiotherapy on stress incontinence treatment and describe the physiotherapy impact on the quality of life.The exercise unit with patients consisted of entry check-up and then the individual therapy was created including exercise methods such as Mojžíšova's, Kegel's and development positions according to Mr. Kolář. Patients used for strengthening the pelvic floor the Kegel8? system. Finally, the examination and evaluation was performed. The quality of life was determined through interview and CONTILIFE questionnaire, which was submitted at the beginning and the end of the therapy. Final results showed that the chosen therapy was not as successful as expected. Only one patient felt the improvement of her incontinence. Other patients did not feel any difference. Following to the results, the quality of life remained almost intact. The reason behind the result could be the inactivity of patients and short duration of the therapy. However, all patients experience improvement in their physical condition, including individual problems that seem to be unrelated to stress incontinence.

Influencing of stress urinary incontinence of women by different physioterapy methods

The topic of my bachelor's thesis is affecting the women stress incontinence using different physiotherapeutic methods. The thesis consists of a theoretical part and a practical part. The theoretical part covers the problematic of physiology and anatomy of the urinary system and pelvic floor. It also explains the theory of incontinence, diagnosis and treatment of the stress incontinence. Stress incontinence is a state in which one cannot intentionally regulate the urine drain using increased intrapelvic pressure. It is one of the most common women diseases. It is a multifactorial problem, because it causes not only physical, but also mental, social and even economic problems. Stress incontinence is a symptom which can have several causes. Under normal circumstances, the urethra reacts on increase of the intrapelvic pressure by increasing its own pressure and thus it is closed. Therefore it cannot leak the urine. If, however, the urethra is not in its physiologic place, which can be caused by a pelvic floor muscle malfunction, the urine is being leaked uncontrollably. The pelvic floor muscles are crucial part of the core stabilization system. They are very important stabilizers of the whole torso and together with diaphragm they ensure the proper breathing. The stress incontinence can be treated also using conservative and surgical methods. In most cases, the incontinence is treated using only passive methods, by using medicines and applying incontinence tools. In the worst cases, they are treated by a surgery. Women, however, have a possibility to treat the incontinence by physiotherapy. Using this treatment, the symptoms of incontinence can be eliminated or at least reduced to a manageable level. The incontinence can be prevented by doing particular exercises. The goal of the physiotherapy is to teach the women how the urinal system and mainly the pelvic floor work. To help them with the training of the activation and relaxation of the pelvic floor muscles and how to include them during the breathing together with the diaphragm. It is important to focus on the woman mentality. We are trying to support the proper self confidence of the women. The women have to be treated comprehensively and individually. The practical part of the thesis was done using qualitative research. Two patients suffering from stress incontinence were treated during the research. Based on the entrance kinesiologic analysis, an individual exercise plan was created for each of the patients. In the end, the output kinesiologic analysis was done. Mainly, the postural therapy, the flat foot therapy, techniques by Capova, therapy by Ludmila Mojzisova, etc. were used. It is clear from the results that the used therapy was successful and it was rated very well by the patients. According to the patients, the exercises had influence not only on their incontinence problems, but it also helped to improve their overall condition, both physical and mental.
Stress incontinence and the possibilities of targeted rehabilitation

The topic of my bachelor thesis is stress incontinence and options of targeted physiotherapy. The first section contains theoretical background concerning mentioned problems. The theoretical section summarizes anatomical and physiological facts about urinary system and pelvic floor, and the theory of incontinence. There are described types of urinary incontinence and their causes. As next, there are mentioned the options of diagnostics and treatment especially of stress incontinence. Incontinence, i.e. involuntary leakage of urine, is one of the most widespread female inconveniences of recent years. It brings many problems, not only physical ones but also mental, and so that it has negative impact on quality of the life. This involuntary leakage of urine is not an illness to all intents and purposes but symptom with various causes. The most common cause are weakened muscles of pelvic floor, which belong to the deep stability system. They significantly support the whole stability system of the trunk. These muscles and fasciae of pelvic floor provide proper stabilization of urinary bladder, urethral closure, and urethra in proper position and by this they assure continence, especially when intra abdominal pressure increases. The muscles of pelvic floor and diaphragm participate together in breathing. The frequency of incidence of urinary incontinence increases depending upon the age. One of the incontinence types is stress incontinence. Urine leakage is always related to physical activity. Urine leakage happens when intra abdominal pressure increases and happens without contraction of detrusor. Unlike against other disorders, it is possible to fight successfully against incontinence not using only drugs and operative surgery. The treatment of incontinence is in recent years relatively successful and with the help of proper and regular exercise it is possible to prevent it. According to some recommendations, when treating stress incontinence, physiotherapy should be chosen as a primary treatment. The goal of physiotherapy is the effort to help patient to be able to activate muscles of pelvic floor in situations when the leakage of urine is expected. There was used the method of qualitative research in practical section of the thesis, using case studies of 2 patients suffering from stress incontinence. The thesis contains entry and final kinesiologic analysis completed with the query CONTILIFE, the plan with adequate exercises and measures, and the final success rate of the treatment. The goal of the thesis was to point out significant role of physiotherapy in the treatment of patient with stress incontinence, then outline the available rehabilitation methods and propose complex of appropriate exercises and measures. There was created a booklet with the list of exercises for each patient. Patients have exercised using these single exercises being previously instructed about them for eight weeks, and they were simultaneously controlled on regular basis. Patients were observed in a complex manner so that the individual therapy was made for each of them according to individual problems, revealed in entry kinesiologic analysis. Final results of this study showed that the chosen therapy was successful and patients mention not only subjective feeling of improvement of their incontinence problems during the day, but also an improvement of their physical condition and relief from the pain of the cervical and lumbar spine due to a complex approach and individual therapy.
